What is PCOS and why does it make me break out?

Polycystic Ovarian Syndrome, or PCOS, is a common but often missed hormonal condition that affects between 6 and 20% of women. Whilst the symptoms vary from one person to another, up to 75% of women with PCOS suffer from acne.
So what is PCOS, why does it cause acne and, most importantly, what can you do about it?
What is PCOS?
Thanks to its name, you’d be forgiven for thinking that PCOS is a disease of the ovaries. Whilst some women with PCOS do have multiple cysts in their ovaries, not all do, and many women have similar looking ovaries but don’t have PCOS. So what gives?
The true culprit in PCOS is not the ovaries, but hormones. Specifically, an excess of androgens, or hormones often associated with men but women have them too, and high levels of insulin with associated insulin resistance.
We don’t know yet what the cause is, but it’s probably a combination of genetics and lifestyle factors.
What are the symptoms of PCOS?
Whilst it’s different for everyone, the most common symptoms are:
- Irregular periods
- Acne
- Hirsutism (excess hair growth)
- Fertility issues
- Weight gain
- Mental health effects like depression and anxiety
Diagnosis involves a trip to your doctor and, if the symptoms are suggestive, some blood tests and maybe an ultrasound of the ovaries.
But why am I breaking out??
There are several steps on the pathway to PCOS acne. And just to complicate matters, it’s a circular pathway, with every step contributing to all the others. Hello vicious circle.
Oilier Skin: The skin’s sebaceous glands, where sebum, AKA oil, is produced are very sensitive to androgen hormones. When the androgen levels go up, so does the sebum production and voila, oily skin, one of the key drivers of acne. The composition of the oil changes too, with different types of lipids taking over which can mess up the skin barrier.
Bad Bugs: Cutibacterium acnes (known as C. acnes to its friends) is a skin bacteria that we know is one of the primary drivers of acne – but C. acnes is part of the normal flora of the skin and doesn’t cause any problems for people without acne. So why is it acting up in PCOS skin? Here again, we have the excess and altered make-up of the sebum providing ideal conditions for these nastier strains of C. acnes to flourish, which in turn kill off the good bacteria that normally keep it in check. Other virulent strains of usually harmless bacteria can also join the party and we are now well on the road to inflammation.
Comedone (Pimple) City: C. acnes and its accompanying inflammation lead to hyperkeratinisation, or overproduction of the superficial skin cells, along with a slower skin cell turnover. Hello blocked pores, causing pimples and blackheads, AKA comedones, and more inflammation. AND (just in case there wasn’t enough bad news) this all sets off abnormal immune responses in the skin, triggering more inflammation, growth of C. acnes and its diabolical germ friends.
Please tell me some good news! What are the treatment options for PCOS?
Whilst there is no cure for PCOS (yet) there are a LOT of treatment options to bring things under control:
Lifestyle changes: Maintaining a healthy weight with a balanced diet and daily exercise can help regulate menstrual cycles and improve insulin sensitivity. Managing stress and getting enough sleep can also help with mental health issues and it certainly won’t hurt your skin either!
Medications: Because PCOS is basically a hormone problem, there are several medications that can help:
- The oral contraceptive (birth control) pill reduces androgen levels, which regulates periods and controls acne.
- Metformin, a treatment for type 2 diabetes, works by improving sensitivity to insulin. We now have good evidence that it helps with many of the PCOS symptoms including acne.
- Other anti-androgen medications can manage unwanted hair growth (hirsutism) and acne. These can be an option for people who can’t take the contraceptive pill.
Which brings us to the skin..
Whilst PCOS medications will go a long way to helping your skin, you still need a skincare regime with proven active ingredients that target the main culprits:
